What could be the reason for the failure of the rectifier bridge when the switching power supply is turned on? How should it be solved?

LED driving power supply, 35W, 220V input, the rectifier bridge breaks down at the moment of startup, and the breakdown position is at the negative pole. The rectifier bridge specification is that the inrush current tested by 3A1000V is not big, only a dozen A.

1. What could be the reason for the failure of the rectifier bridge at the moment of power-on? (Analyzed the failed rectifier bridge is caused by overcurrent)

       Personally think it is: a. The quality of the rectifier bridge itself. b. The circuit has a short circuit after rectification (but the same bridge is replaced, and the power supply is fine).

What else is there?

2. The rectifier bridge is applied to the switching power supply. Is the two crystal grains at the negative position among the four crystal grains more likely to fail than the one at the positive position? why?

Can you take a look at the circuit of your power supply input terminal, it should be insufficient buffering?
